Please allow 5-7 days when requesting Medical Records.
There is no charge to the patient for a copy of their records. Nor is there any charge for records sent to treating physicians.
Records sent to attorneys, insurance companies and individuals other than the patient may be charged $1 for the first page, 50 cents for each additional page and require a signed release from the patient. Please click on the link below to obtain a copy of the release form.
